Return to this registration page and proceed with the link below.  You will need the USA Hockey number that you received in Step 1.

Step 4: Submit Forms

• Birth Certificate:  All players new to CHA, must email a pic (no pdfs) of your child’s birth certificate to CHA Registrar:   Registrar@ChelseaHockey.org

Payment

• PAY ONLINE IN FULL: Submit total CHA Registration fees online upon conclusion of the registration session via Visa, MasterCard, Discover or checking account.

REFUNDS & PRO-RATED FEES
 

• REFUNDS & PRORATED FEES: Any request for refund, due to a player leaving a program early, or a pro-rated fee, due to a player signing up late in a program, must be submitted in writing to the CHA Board. The Board shall review all written requests for refund or pro-rated fee and notify the participant of its decision on an individual basis.

COVID-19 POLICY
 

In the event the youth hockey season is cancelled or shortened as a result of the COVID-19 pandemic any refunds will be provided based upon the following:

1. The cancellation or shortening may be required by any of several entities including by not limited to: the State of Michigan, USA Hockey, Michigan Amateur Hockey Association (MAHA), District 6 Hockey, other local government regulations.

2. Certain costs incurred by CHA are non-refundable in nature and often occur prior to the start of the season.  These costs are normally covered by player registration fees collected annually.  Examples include but are not limited to: team equipment, jerseys, tournament registrations, technology costs, accounting fees, coaching fees and supplies.

3. Any refund approved will be based upon CHA's costs incurred to date and the portion of the season completed.
