ALL PLAYERS ARE REQUIRED TO SUBMIT:

1. Copy of birth certificate
2. Copy of USA Hockey Registration (please register with the name as shown on the birth certificate)
3. Player Conduct Form
4. Parent Conduct Form (signed by both parents - required)
5. Medical/Insurance Form

REGISTRATION FORMS ARE AVAILABLE UNDER DOCUMENTS.

Please contact appropriate travel team coaches for try out information. Check under Teams for the travel team websites and calendars.

PRINT ERROR - the mailed copies of the 09-10 Information had incorrect birth years listed (page 2), however, the Registration Form does have the correct birth years listed and the online Information form has been corrected. We apologize for any confusion.

PRIORITY REGISTRATION INFORMATION

Early registration for the 2009-2010 season has started. We only ask for the $150 registration fee at this time. Registrations received without the registration fee are put on a wait list until that fee is received.  We encourage everyone to create an account on this site and register online - it will make the job much easier for our volunteers.  You may register and pay online or by mail.

All '96, '97, '98 players welcome.  $20/session or $140/ all 9 sessions. See full details in link below.

All CHA members are requested to create an account on the CHA website in order to obtain additional access to information and for managers to grant permission to specific team information.

Team managers are to create an account on the CHA website, then email Peter Heath to set up a time to get orientated to the new website.

Home ice for the CHA programs is at the Arctic Coliseum, located at 501 Coliseum Drive just off M-52 and Old Manchester Road. You can reach the CHA for questions by calling 734-433-9665 or you can send an email to ej@chelseahockey.net.
